Each boy who attends Brigade must have a permission slip and medical release form signed by their parent/guardian indicating that they are allowed to attend and join the program.

You may pre-register for Brigade at the beginning of each school year by filling out the online pre-registration form below (or if you prefer, download the pre-registration form, fill it out, and mail/scan it in).

This form must be filled out by the parent/guardian of each child attending. Please fill out this form and return it by hand to the leaders of Brigade.
